Post-Acceptance Requirements
Incoming students must complete post-acceptance requirements by November 1st, unless noted otherwise in the seat offer letter. There are three (3) parts to the post-acceptance process:
- Sign post-acceptance forms.
- Sign up for and complete a post-acceptance account.
- Document the completion of outstanding academic requirements, if applicable
Step One: Post-Acceptance Forms
Forms will be sent electronically after a student makes a seat deposit. Students will receive the following forms:
- The Technical Standards Attestation Form1
- The Consent to Share Information Form
- The Authorization for Release of Information Form2
- The Human Subject Consent Form
1All students must meet the minimum technical standards with or without reasonable accommodation necessary to achieve the knowledge, skills, and competencies required for an entry-level physician assistant. All candidates who are offered a seat in the program will be required to sign a statement attesting to their ability to meet these technical standards.
2Each clinical site where students are assigned for their supervised clinical practice experiences requires a copy of the student’s immunization history, titers, and tuberculosis screening results. In order to facilitate the provision of this information to the clinical sites, the PA program requires admitted applicants to sign a health information release form, which authorizes the program to maintain a copy of this information in a student record and release copies of this information to the clinical sites where students are assigned for training.
Step Two: Student Compliance Account
On October 1st, deposited students will receive an email with instructions to create a compliance account where they submit post-acceptance documents and order background checks. Students are responsible for covering the cost* of this account.
Students will upload to the compliance account:
- A government-issued ID
- Proof of health insurance
- Immunization records per CDC healthcare personnel recommendations (see table 2)
- Tuberculosis (TB) screening results
- Serological titer reports
In addition to complying with the CDC healthcare personnel recommendations, review the UC PA Program’s Student Health Policy (←click here) for program-specific immunizations, Tuberculosis (TB) screening, and serological titer requirements. Students are responsible for all associated costs. *
In the compliance account, students will order and pay* for a background check. If a student has been a New York resident within the past seven years, they will be required to pay* an additional fee for a New York background check. Students may be requested at any time to order and complete a drug screen for an additional cost. *

Step Three: Outstanding Academic Requirements (if applicable)
Students will be notified in the seat offer letter if any academic requirements need to be completed before matriculation. Examples include, but are not limited to, a receipt of an official transcript from a regionally accredited college or university confirming the completion of a bachelor’s degree or higher, in-progress coursework, including prerequisites with the required minimum letter grade, while maintaining a minimum 3.0 cumulative GPA.
