TRANSCRIPT REQUEST PROCEDURE

Transcripts from the University of Charleston/Morris Harvey College may be requested by downloading the University of Charleston Transcript Request Form.  Simply download the form, fill in the appropriate fields, enclose the processing fee, and mail or fax the form to the Student Solutions Center.

OFFICIAL TRANSCRIPT REQUEST FORM

It is essential for you to include the complete mailing address where you would like the Student Solutions Center to mail your transcript.  If you are sending your transcript to another college or university you should contact their Office of Admissions to verify their complete mailing address.  Larger institutions often have very specific mailing addresses to which transcripts should be sent.  Therefore, in order to prevent the transcript from being sent to the wrong department or address and delaying your potential admission, we strongly urge you to contact the institution to verify proper mailing address.  The University of Charleston will mail transcripts to the address(es) denoted on the University of Charleston Transcript Request Form.

Requests are normally processed within 1-2 business days upon receipt of the University of Charleston Official Transcript Request Form and proper payment. Please be sure to fill in all fields and sign the form.  Forms received without a signature from the individual whose name is on the transcript being requested will not be processed in accordance with federal mandates regarding the disclosure of academic records.  Third parties such as spouses, employers, potential employers and parents must have the written consent of the individual whose name appears on the transcript.

The University of Charleston does not accept e-mail or telephone requests because they do not contain written signatures.

The cost of an official copy of a transcript is $4.00.  Please fill out a separate form if you are requesting more than one transcript.
