UserID: Student-Athlete’s school email address (i.e. johndoe@cc.ucwv.edu)
Password: last four digits of social security number

Additional Information
If you were evaluated by a physician over the summer for a medical condition you will need to bring copies of your medical records with you to the team physicals.**You must bring a copy of your insurance card – front and back**
